We examined how curcumin-mediated photodynamic therapy (CUR-PDT) induces a specific type of cell death called ferroptosis in fibroblast-like synoviocytes from rheumatoid arthritis patients.

Folate's Role in Rheumatoid Arthritis
We explored the potential of folate-modified ginger-derived extracellular vesicles (FA-GDEVs) in treating rheumatoid arthritis (RA). This innovative approach aims to tackle the ongoing inflammation and joint damage that characterizes this autoimmune disorder. By using folate, which selectively targets specific receptors on inflammatory cells, we specifically directed these vesicles to M1 macrophages in inflamed joints.

Our findings show that FA-GDEVs can help encourage a shift from harmful M1 macrophages to a healing M2 phenotype. This shift is significant because M2 macrophages play a role in tissue repair and reducing inflammation. We also investigated how these vesicles activate the PI3K-AKT pathway, a crucial cellular pathway involved in regulating immune response.

We explored a unique case involving a 54-year-old man who experienced symptoms that indicated both Guillain-Barré syndrome (GBS) and complications from autoimmune gastritis (AIG). Notably, he displayed progressive numbness and weakness along with gastrointestinal symptoms, which are often linked to vitamin B12 deficiency.

As part of his treatment, we utilized intravenous immunoglobulin alongside methylcobalamin, a form of vitamin B12. What was particularly interesting is that while the immunoglobulin treatment is typically aimed at addressing GBS, vitamin B12 was also administered to combat the neurological effects stemming from the deficiency due to AIG.

Our findings revealed that following these treatments, the patient showed significant clinical improvement before discharge. However, it's crucial to note that while vitamin B12 is commonly understood to aid neurological functions, the specific impact of its isolated use in this case remains unclear without additional studies focusing solely on its effectiveness in autoimmune disorders like GBS.

I took this iron to replace Hemaplex, as I had low haemoglobin which led to weakness and pale skin after suffering from COVID-19. After a month on this supplement, I feel significantly better — my haemoglobin has normalised, I have more energy, and there’s colour back in my cheeks. This iron includes co-factors like B12 and vitamin C for better absorption. The brand is excellent, and I trust it, so I will order more.
We investigated the potential of vitamin B12 as a treatment for rheumatoid arthritis, particularly focusing on its ability to target an enzyme called peptidylarginine deiminase isoform 4 (PADI-4). PADI-4 plays a crucial role in a process called citrullination, which is often disrupted in autoimmune conditions like rheumatoid arthritis.

Our findings revealed that vitamin B12 effectively inhibits PADI-4, showing a preference for this enzyme over another similar one, PADI-2. This inhibition corresponds to a decrease in overall cellular citrullination, including the citrullination of histone H3, which is also mediated by PADI-4.

Additionally, we observed that hydroxocobalamin, a special form of vitamin B12, significantly reduced the severity of collagen type II antibody-induced arthritis in mice. It also lowered the expression of key inflammatory factors and cytokines associated with rheumatoid arthritis, such as IL-17A, TNFα, and IL-6.

These promising results suggest that vitamin B12 might be an effective therapeutic avenue for managing rheumatoid arthritis, warranting further investigation in future studies.

We investigated the role of folic acid supplementation in helping elderly patients with rheumatoid arthritis (RA) who are treated with methotrexate (MTX). This study focused on understanding the incidence of adverse events (AEs) that lead patients to stop using MTX over time. We reviewed clinical data from 198 elderly RA patients who were receiving MTX in an outpatient rheumatology setting.

Over a period of 15 years, we found that the likelihood of patients stopping MTX due to AEs increased significantly, with rates reaching as high as 51.7%. Notably, we observed that higher doses of MTX (15 mg/wk or more) substantially raised the risk of withdrawal. In contrast, patients who took folic acid alongside their MTX therapy were less likely to discontinue treatment, suggesting a protective effect of folic acid against some of the adverse reactions.

Overall, our findings indicate that while MTX is a common treatment for RA, utilizing lower doses along with folic acid supplementation can help manage the risks associated with this medication. This approach could lead to improved long-term treatment outcomes for elderly patients with RA, allowing them to better navigate the challenges of their condition.
We looked into how folate, an essential nutrient in our diet, might influence the health of individuals with systemic lupus erythematosus (SLE), an autoimmune disorder that carries a heightened risk of cardiovascular disease. Our investigation involved a case-control study with 394 Mexican-mestizo women, where we sought to find connections between certain genetic variants related to folate metabolism and heart health in SLE patients.

Surprisingly, our results showed that the overall levels of folate and homocysteine—both important in the context of heart health—didn't differ significantly between those with SLE and healthy controls. However, we did observe that the genetic variant c.+677 C>T (known as the CC genotype) appeared to offer some protective benefit against cardiovascular issues, suggesting that people with this genetic makeup might have a lower risk of heart disease.

On the flip side, two specific haplotypes from the c.+677 and c.+1298 genetic variants were linked to an increased risk for SLE itself. This suggests that while folate's role in the body is vital and its genetic connections intricate, treating SLE patients with folate alone may not yield significant improvements in their condition based on these findings.

Overall, while we documented interesting associations between genetic variants and disease risk in SLE, we didn't find strong evidence supporting direct benefits of folate treatment in mitigating the disorder's impact.
